Job summary

Concord Hospital Health System is seeking a Patient Safety Attendant to provide 1:1 observation for behavioral health patients or those at risk. You will monitor in person or via video, identify safety risks, and alert the team to intervene promptly.

You will document care, maintain boundaries and confidentiality, and assist with basic needs while ensuring continuous supervision and a compassionate care environment.

Qualifications

  • High School Diploma or GED required.
  • American Heart Association Heart Saver certification within 6 months of hire.
  • Management of Aggressive Behavior certification within 6 months of hire.
  • Demonstrates ability to work as a team member and communicate effectively.

Responsibilities

  • Provide 1:1 monitoring with continuous visual observation for patients.
  • Identify risks (falls, self-harm) and alert the team promptly.
  • Document care provided and report observations to the RN.
  • Maintain patient confidentiality and follow hospital policies.
  • Assist with basic patient needs and ensure continuous supervision.

Summary

Under the direction of the Registered Nurse, LPN, or Paramedic the Patient Safety Attendant is responsible for providing 1:1 monitoring with continuous visual observation (either in-person or on a video monitor) for adult or pediatric behavioral health patients or patients at risk for suicide, falls risk altered mental status and delirium.

Under the direction of the Registered Nurse, LPN, or Paramedic the Patient Safety Attendant is responsible for providing 1:1 monitoring with continuous visual observation (either in-person or on a video monitor) for adult or pediatric behavioral health patients or patients at risk for suicide, falls risk altered mental status and delirium. The role will focus on providing a safe and comfortable environment for those patients at risk of harming themselves or those with altered mental status.

Patient Safety Attendant Will Be Able To Demonstrate
  • Sensitivity and respect for the patient, family and/or significant other(s)
  • A calm, confident manner.
  • Identifies risks such as falls or self-harm and promptly alerts the team to intervene early.
  • Good communication, listening and observation skills.
  • When assigned to the 1:1 observation of a patient, does not leave the patient until relieved by another staff person; never leaves the patient unattended unless directed by the nurse.
  • Documents all pertinent information
  • Adheres to hospital policies including but not limited to bed/chair alarms, fall prevention, delirium and safe patient handling
  • Maintains infection prevention standards
  • Collaborates closely with the healthcare team to foster a culture of compassion, respect and teamwork
  • Supports and participates in the organization’s continuous quality improvement program
  • Recognizes a patient emergency and respond appropriately
  • Assist in preparing patient meal tray for patients as directed by the RN
  • Assist with ambulation and mobilization activities for those patients that are independent

Available to work shifts including day, nights, weekends and holidays

Education

High School Diploma or equivalent (GED)

Certification, Registration & Licensure
  • American Heart Association Heart Saver within 6 months of hire

Experience

Demonstrates ability to perform as a team member and effective communicator. Must be 18 Years of age or older

Responsibilities
  • Communicates with patients and significant others in a respectful manner.
  • Orients patient and/or significant other(s) to situation and surroundings.
  • Documents care provided and reports to the RN observations of patient’s physical, social and mental status.
  • Maintains appropriate boundaries with patients.
  • Always ensures continuous supervision of patients. The usage of cell phones, laptops, books, etc. is strictly prohibited.
  • Maintains confidentiality of patient information.
  • Adheres to relevant policies, procedures and standards.
  • Assists patients with basic needs such as ordering meals, eating, mobility and bathroom assistance for appropriate patients
