Job summary
Sevita is seeking a Licensed Practical Nurse (LPN) at our Williston, TN site to support individuals with intellectual and developmental disabilities. You will provide nursing services, training, and supports, monitor health conditions, administer medications, and coordinate care with the ISP team.
The role requires an Associate’s Degree in Nursing or state certification and active LPN/LVN licensure in the state of residence, plus CPR/FA and HIV/AIDS certifications.
Qualifications
- Associate’s Degree in Nursing or state certificate.
- One year of experience in nursing with the target population.
- Current LPN/LVN licensure for the state of residence.
- Current driver’s license, car registration, and auto insurance.
- CPR/FA and HIV/AIDS certification or equivalent.
- Ability to communicate effectively with staff and individuals served.
Responsibilities
- Monitor health conditions and ensure quality services.
- Administer medications and document appropriately.
- Oversee and coach staff on medication administration.
- Provide physician-directed medical treatment and personal care services.
- Conduct health assessments and update ISP plans.
- Train staff on safety, infection control, and medication procedures.
- Coordinate with providers and external agencies for seamless care.
- Educate individuals served on self-management and health topics.
