Job summary

Fresenius Medical Care is seeking an entry level Registered Nurse for the Outpatient CAP1 program. You will provide and coordinate clinically competent care, including assessment, planning, intervention and evaluation for a group of patients undergoing hemodialysis.

Responsibilities include data collection, patient education, safe delivery of dialysis therapy, medication administration, emergency response, and collaboration with the interdisciplinary team to update care plans and outcomes.

Qualifications

• Entry level for RNs with less than 2 years of Nephrology Nursing experience.
• Minimum 9 months experience as a RN.
• Minimum 3 months experience in chronic/acute hemodialysis as a RN.

Responsibilities

• Perform essential functions under supervision and with guidance from nursing leadership.
• Collect and analyze patient data before, during, and after hemodialysis and document in medical records.
• Educate patients and families on learning needs and treatment plans.
• Deliver safe and effective hemodialysis therapy with the team.
• Administer medications as prescribed and document justification and effectiveness.
• Initiate or assist with emergency response measures.
• Serve as resource for team and participate in training of staff.
• Ensure proper lab collection, processing and reporting; reschedule missed collections.
• Update nursing assessment and care plans in collaboration with the interdisciplinary team.
• Discuss transplant and treatment modality options with patients.
