Job summary

Montfort Care in Singapore is seeking a Central Triage Unit Social Worker/Case Worker to shape the first experience for individuals and families seeking support. The role uses structured assessments and professional judgement to determine appropriate support pathways across Montfort Care services and community partners.

You will manage enquiries, conduct risk assessments, coordinate referrals, document cases, and ensure confidentiality while maintaining a compassionate, person-centred client

Qualifications

• Recognised degree or professional qualification in Social Work or related field.
• Experience in triage, intake, needs assessment, information and referral, or case coordination.
• Fresh graduates with good interpersonal skills welcome.

Responsibilities

• Triage enquiry and intake: receive and manage enquiries via phone, email, walk-ins, forms and referrals; conduct initial triage and intake assessments.
• Safety and risk assessment: screen for family violence, child safety concerns, self-harm, suicide risk, elder abuse; initiate safety planning.
• Service navigation and coordination: identify and refer appropriate services; facilitate referrals and warm handovers; coordinate with partners.
• Client experience: provide responsive, empathetic and person-centred service from first contact; guide clients through options and next steps.
• Case documentation and system administration: create and maintain cases; complete intake, referrals, risk and case documentation; ensure confidentiality.
• Undertake any other duties as assigned by supervisor.
