Responsibilities

• Establish strong rapport with patients, families, and care partners.
• Be main contact for care coordination and navigation.
• Assist in screening patients for health and social care needs (distress thermometer, vital signs, caregiver burden).Escalate to medical/nursing when appropriate.
• Assist in collating screening forms and populating database.
• Monitor patient conditions through calls or home visits and coordinate care services.
• Maintain updated care plan documentation if required.
• Educate patients and families on self-management and healthy lifestyle.
• Support community outreach and engagement efforts.
