Working Days & Hours: * 5.5 Days (Mon – Fri: 8.30am – 6pm, Sat: 8.30am – 1pm)

Job Description

• Patient Care: Assess, plan, implement, and evaluate evidence-based nursing care to meet the needs of patients.
• Chronic Disease Management: Develop and provide independent nurse‑led services in chronic disease management and preventive care.
• Education: Educate patients and their families about disease prevention, management, and rehabilitation techniques, as well as train caregivers for basic care provision.
• Collaboration: Work collaboratively within a multidisciplinary team to ensure integrated care and effective communication with other professionals/agencies.
• Supervision: Supervise and coach a team of junior nurses (if a senior position) to provide optimal care and service to patients.
• Documentation: Maintain accurate, complete, and contemporaneous nursing records.
• Standards & Compliance: Adhere to professional nursing practice standards, Trust policies, procedures, and clinical guidelines, practicing at all times in accordance with the Singapore Nursing Board (SNB) Code of Professional Conduct.
