Responsibilities

• Participates in planning and delivery of patient care in haemodialysis.
• Conducts patient assessments pre, during and post dialysis.
• Adheres to FMC Policies related to clinical and operational management.
• Reviews blood test results for abnormalities.
• Educates patients about diet, fluid, and medications.
• Motivates patients for adjustment to chronic hemofiltration.
• Monitors dialysis water treatment systems.
• Establishes effective communication with patients and relatives.
• Promotes a safe and healthy working environment.
• Ensures equipment cleanliness and safe operation.
• Maintains accurate patient medical records under confidentiality.
• Participates in quality improvement processes.

Job description

Patient Care

• Participates in the planning, delivery and evaluation of patient care within the haemodialysis clinical setting:
• Undertakes clinical/patient assessments pre, during and post dialysis treatment to evaluate effectiveness of care
• Adheres to all FMC Policies and Work Instructions related to clinical and operation management
• Reviews blood test results and reports to Medical Director any abnormalities
• Educates patients on their diet, fluid, medications and blood pressure management to optimise their clinical outcomes
• Motivates and encourages patient’s adjustment to CHD
• Monitors the water treatment system and fluid for dialysis to ensure provision of safe, quality dialysis therapies
• Establishes an effective relationship with patients and maintains good communication with patients and relatives; and gives adequate support.
• Ensures/promotes a safe, secure and healthy working environment to staff and a comfortable, safe environment for visitors and patients as detailed in the IPC and environment policies
• Ensures all equipment is clean, safe and in good working condition; and reports defects through appropriate channels
• Maintains accurate patient information in medical records, including Therapy Data Management System (TDMS)/EuClid Clinical Information system and abides by confidentiality and Privacy Policy.
• Protects the rights of patients by adhering to Privacy Principles and providing an excellent standard of care
• Actively participates in the quality improvement process by reporting all incidents, hazards and opportunities for improvement
