Job summary

THYE HUA KWAN MORAL CHARITIES LIMITED seeks a dedicated social worker to join the EIPIC THK DS-LS team. The role involves case management, family engagement, and coordinating holistic support for children with mild learning needs in preschools.

You will conduct parent meetings, prepare reports, collaborate with community agencies, and ensure documentation is accurate. Travel between centres may be required, and a recognised social work degree with 1–3 years of relevant experience is preferred.

Qualifications

  • Minimum Degree in Social Work from a recognised institution.
  • 1 to 3 years of experience working with families and children preferred.

Job description

The Early Intervention Programme for Infants & Children (EIPIC) division helps equip children with motor, communication, social, self-help and cognitive skills. THK Development Support and Learning Support Programme (THK DS-LS) is a community-based intervention programme for K1 and K2 children with mild learning needs. We provide short-term support to children in their preschools in order to optimise their learning as well as increase their participation and integration in daily classroom activities and routines.

Reports to the Senior Social Worker, HOD (Social Work), and/or DS-LS Programme Manager.

Roles and Responsibilities:
Case Management
  • Conduct social work-parents meeting to share assessment outcomes of child and recommendations/next steps required
  • Facilitate parents' understanding of programme and needs of child
  • Establish rapport, engage parents and conduct holistic bio-psycho-social assessment of child's and family's needs, risks and strengths
  • Evaluate and identify areas for follow up (e.g. referral to internal/external resources)
  • Ensuring reports and case recordings are up to date.
Documentation and administrative tasks
  • Participate in Case Filter Meetings, take notes and maintain good record keeping
  • Complete documentation (e.g. case notes) accurately and promptly for other professionals who are working with child
  • Maintain accurate records, prepare reports for governing agencies, collate information and submit government means-test applications
Collaborative Partnerships/Community Network
  • Demonstrate knowledge of available community resources
  • Engage and work collaboratively with various community agencies (e.g Family Service Centres, Social Services Offices or other agencies) to ensure holistic case management for the child and family.
  • Contribute social work assessments, perspectives, and professional recommendations during multidisciplinary and inter-agency discussions to support integrated and strengths-based intervention planning.
  • Coordinate care plans and follow-up actions with internal and external partners to ensure integrated support for clients.
  • Implement intervention plans collaboratively with the team members/community to address clients' developmental, behavioural, or social-emotional needs.
  • Build and sustain constructive professional relationships with community stakeholders to enhance service integration and outcomes for clients.
Qualifications
  • Minimum Degree in Social Work from a recognised institution
  • 1 to 3 years of experience working with families and children preferred
