Job summary

THE SUPREME HR ADVISORY PTE. LTD. in Tampines seeks a social services professional to lead outreach, referrals, and case assessment activities within the Division.

The role offers a 5-day work week (Mon-Fri), 9am–6pm, with a salary of SGD 4,700– SGD 5,300 per month. You will collaborate with clients to develop intervention plans, provide counselling, document records, and support ongoing research and professional development.

Qualifications

• Degree in Social Work or equivalent with 2 years of experience in social service sector or similar setting.
• Proficient in MS Office and social work interventions; developing clinical skills.

Responsibilities

• Initiate outreach plans to publicise Division Programmes and build stakeholder relationships.
• Manage referrals and engage referral agencies for intake and assessment.
• Conduct case assessments using intake information and stakeholder input.
• Collaborate with clients to develop intervention plans and provide counselling.
• Document all case records accurately and timely.
• Support research projects and stay updated on current trends.
• Prepare for and participate in case reviews and supervise interns as needed.
• Develop and implement new projects aligned with Division objectives.

Responsibilities

• Initiate and/or participate in outreach efforts related to the Division, which include developing outreach plans to publicise the Division Programmes and establishing good relationships and contacts with stakeholders or community partners.
• Management of referrals, and engaging referral agency / person for intake and assessment.
• Case assessment and conceptualisation by conducting a thorough assessment of the case using information gathered during the intake and subsequent engagement with client, family and stakeholder.
• Collaborate with clients to develop intervention plans, and provide counselling and support to address their risks and needs issues; advocate for clients' rights and access to essential services within the community and relevant systems.
• Documentation of all case related records.
• Research and evaluation through keeping abreast of current research and trends, and participate in and support research projects.
• Professional development and education by preparing for, attending and presenting cases at Internal Case Reviews / Case Conceptualisation meetings / Clinical supervision on a regular basis. Involve in supervising interns and/or students on placement
• Develop and implement new projects/initiatives that are aligned with the objectives of the Division.
