Overview

Social Worker

Salary: $3500 - $4500
Working Days: Mon – Fri (40hrs/week, center operations hours from 8am to 6pm)
Working Location: Choa Chu Kang / Telok Blangah

Responsibilities
  • Review applications from AIC. Interview clients and caregivers during enrolment into our programme/Home. Conduct biopsychosocial assessments to assess the strength and resilience of the client, families, and social support systems.
  • Build rapport with residents/clients and their families. Maintain constant communication with clients while addressing their concerns and goals, educate family members on the physical and psychosocial needs of the clients, and help them access internal and external resources.
  • Provide casework management for clients and coordinate with external agencies such as hospitals and polyclinics for clients who require financial assistance for their medical appointments.
  • Develop and implement individual care plans (ICP) for clients and provide therapeutic support as needed, working with other multidisciplinary team (MDT) members.
  • Execute the ICP and monitor progress. Develop and implement programmes and activities in a planned/structured manner aligned to the company’s values to meet clients’ care needs.
  • Coordinate with PT and OT departments to assist in helping clients feel comfortable and confident.
  • Prepare sixth-monthly ICP review reports and monthly review records for clients.
  • Evaluate existing, new and prospective clients based on their needs, desires and limitations.
  • Update clients’ particulars and social reports/reviews. Conduct casework evaluation and maintain case notes/documents.
  • Provide person-centred counselling for clients and/or families dealing with poor mental health, coping and adjustment difficulties in their transition.
  • Perform risk assessment, including assessment of risk of self-harm and risk to others.
  • Financial assessment, fund management, and prepare periodic reports as required. Identify and refer cases for financial assistance (e.g., generate Means Test Subsidy Change Rate Report from MOH for invoices printing/Changed Subsidy report).
  • Any other tasks assigned by the Management.
