Qualifications

• Possess Degree in Social Work or Degree with a Graduate Diploma in Social Work from an accredited institution
• Accredited as Registered Social Worker with Singapore Association of Social Workers would be an advantage
• Prior experience as a social worker would be an advantage

Responsibilities

• Conduct casework management, including gathering information, psycho-social assessment, and identifying needs and goals for intervention
• Provide casework and counselling to persons facing mental health conditions, dementia and their caregivers
• Plan, organise and implement community outreach activities, programmes and group work
• Perform evaluation and prepare reports for administrative investigation and intervention programmes
• Provide information and referral services to link clients to appropriate social services
• Participate in research

Job Description
