Job summary

TOUCH COMMUNITY SERVICES LIMITED in Singapore is seeking a Night EHPC Care Worker to deliver Enhanced Home Personal Care to frail elderly clients during 8:00 PM–8:00 AM, providing short-duration personal care and emergency support. You will respond within one hour to unscheduled care, assess on-site, maintain a safe home, handle wound care and toileting, and document all services for timely reporting.

Qualifications

• Minimum GCE N Level or equivalent; training in at least one of the following: WSQ Higher Certificate, Advanced Certificate in Healthcare Support (Nursing Care), or ITE Approved Training Centre – Healthcare (Home Care) Programme.
• For Local Applicants only: WSQ/HCA/ITE healthcare qualifications; For Foreign Applicants: Nursing Aide course plus caregiving experience preferred.

Responsibilities

• Respond to unscheduled care requests within one hour.
• Conduct on-site assessment and provide appropriate personal care support.
• Maintain a safe, clean and hazard-free home environment.
• Clean immediate living environment when contaminated with urine, stool or other bodily fluids; change linens when soiled at night.
• Provide personal care support: hygiene, assisted shower, toileting, dressing, lifting and transfers.
• Attend scheduled care interventions per care plan (housekeeping and personal care) with medication monitoring.
• Monitor vital signs and support medication adherence.
• Perform higher care tasks: wound dressing, stoma care, PEG tube care, nebuliser, tracheotomy care, and documentation.
• Document all services and report significant changes in client condition.
• Provide admin support and maintain cleanliness of care outposts; perform any ad-hoc duties as assigned.

Job description

Job Summary:

Delivers Enhanced Home Personal Care (EHPC) services to frail elderly clients to support safe ageing-in-place within the community. The role provides responsive, short-duration personal care and emergency support services during night hours (8:00 PM to 8:00 AM). This includes supporting existing.

Enhanced Home Personal Care (EHPC) clients through scheduled night interventions, urgent unscheduled care requests with a one-hour response time, as well as attending to community emergency activations.

The position requires the ability to work independently, exercise sound judgment in urgent situations, and ensure timely escalation and reporting where necessary to safeguard client safety and well-being.

Principal Responsibilities and Duties:

Delivery of Enhanced Home Personal Care services for both scheduled and unscheduled care needs

• Respond to unscheduled care requests within one (1) hour of receiving the call
• Conduct on-site assessment and provide appropriate personal care support
• Maintain a safe, clean and hazard-free home environment

1. Clean immediate living environment when contaminated with urine, stool or other bodily fluids
2. Change linen or bedsheets when soiled during the night

• Provide personal care support to clients

1. Hygiene care, assisted shower or bed sponging
2. Change clothes or diapers
3. Assist with toileting
4. Lift, transfer and reposition clients safely

Scheduled Support

• Attend to scheduled care interventions as per assigned care plan (housekeeping and personal care)
• Medication & Monitoring

1. Assist and remind clients with medication
2. Monitor medication compliance
3. Monitor vital signs (temperature, BP, pulse)

• Higher Care Tasks

1. Simple wound dressing
2. Stoma care
3. PEG tube care
4. Nebuliser with normal saline
5. Hypo-count and documentation
6. Care for clients with permanent tracheotomy
7. Apply cold compress as instructed

• Document all services provided and report any significant changes in client condition

Other responsibilities

• Provide administrative support
• Maintaining cleanliness of care outposts
• Support operational needs of the organisation as required
• Perform any other ad-hoc duties as assigned by Supervisor or Management.

Essential Qualifications:

For Local Applicants

• Minimum GCE N Level or equivalent and
• Training in at least one of the following:

1. Workforce Skills Qualifications (WSQ) Higher Certificate or,
2. Advanced Certificate in Healthcare Support (Nursing Care) or,
3. ITE Approved Training Centre – Healthcare (Home Care) Programme

For Foreign Applicants

• Completed more than 6 months Nursing Aide Course from home country and,
• Relevant caregiving or healthcare experience preferred
