Sengkang General Hospital is seeking a Pharmacist to contribute to a multidisciplinary healthcare team. You will identify and resolve drug-related problems, recommend appropriate drug therapy, and participate in inpatient rounds, medication reconciliation, dispensing, and patient counseling to ensure safe and effective outcomes.

You will also communicate drug information, educate the public, patients, students, and colleagues on safe medication use, and participate in research and quality

Formación

• Degree in Pharmacy and registered as a Pharmacist in Singapore, or eligible for registration with the Singapore Pharmacy Council.
• Good listening and communication skills
• Enjoys interacting with people of all levels

Responsabilidades

• Participates in regular inpatient clinical rounds.
• Performs medication reconciliation and dispensing.
• Counsels patients on their medications and treatment.
• Provides drug information to patients, public, students and healthcare workers.
• Involves in research and quality improvement in the section.

You will be part of a multidisciplinary healthcare team intervening in acute or chronic diseases to maintain current health and promote patients' well-being. You will identify and resolve drug-related problems as well as recommend appropriate drug therapy to ensure safe and effective drug distribution and outcomes. These include active participation in regular inpatient clinical rounds, medication reconciliation, as well as dispensing medications and counselling patients on their medication treatment. You will also communicate timely and accurate drug information, and provide education to the general public, patients, students, and fellow healthcare workers on matters pertaining to safe and effective medication use. In addition, you will be involved in research/ quality improvement efforts in the section.

Requirements

• Degree in Pharmacy and registered as a Pharmacist in Singapore, or eligible for registration with the Singapore Pharmacy Council.
