Job summary

The Supreme HR Advisory Pte Ltd is seeking a dedicated social work professional for a full-time role at Bukit Batok or Buangkok. You will develop individualised care plans for residents, execute and monitor progress, and design programmes to meet residents' needs.

You will also prepare six-monthly reviews and monthly interview records for about 50 residents, contributing to ongoing quality care within the organisation.

Qualifications

Degree in Social Work or Graduate Diploma in Social Work.
Training provided for junior executive.

Responsibilities

Develop individualise care plans for residents
Execute the care plan and monitor progress
Develop and implement programmes and activities to meet the residents’ care needs
Put up 6th monthly review reports and monthly interview records for about 50 residents
