Job summary

Vanguard Healthcare Pte Ltd is seeking a Programme / Rehabilitation Executive to support day-to-day operations at the Psychiatric Day Centre in Hougang.

You will engage clients, monitor progress, conduct assessments, and collaborate with an Occupational Therapist to implement activities within a multidisciplinary team.

Qualifications

• Diploma or Degree in Social Work, Psychology, Nursing, Counselling, or related fields.

Responsibilities

• Build rapport with clients and encourage active participation in meaningful activities.
• Conduct client assessments and maintain accurate, complete, and up-to-date documentation.
• Work with Occupational Therapist on the implementation of activities.
• Coordinate referrals and follow-ups with healthcare providers and social service agencies.
• Responsible for administrative and billing-related matters such as clients transport claim, billing and means-testing (if applicable).

The Programme / Rehabilitation Executive supports the day-to-day operations and programme delivery of the Psychiatric Day Centre. The incumbent facilitates client engagement, monitors clients' progress and well-being, conducts assessments and supports individual with mental health conditions.

Responsibilities:

• Build rapport with clients and encourage active participation in meaningful activities

• Conduct client assessments and maintain accurate, complete, and up-to-date documentation

• Work with Occupational Therapist on the implementation of activities

Requirements:
