Job summary

P CONNECT SERVICES PTE. LTD. is seeking a qualified physiotherapist to provide client-centred care in home and community settings.

You will conduct assessments, design and implement personalised treatment plans, and work with a multidisciplinary team to improve mobility and independence. The role involves educating clients and caregivers, documenting outcomes, and participating in community outreach and training activities.

Qualifications

• Diploma or degree in Physiotherapy.
• Full registration with the Allied Health Professions Council in Singapore.

Responsibilities

• Conduct comprehensive assessments of clients' physical, functional, and mobility status.
• Develop and implement individualised physiotherapy intervention plans based on clinical findings and client goals.
• Provide evidence-based therapy to improve strength, balance, endurance, and movement efficiency.
• Educate clients and caregivers on exercise programmes, fall prevention, and safe mobility techniques.
• Document assessment findings, treatment plans, progress, and outcomes accurately and in a timely manner.
• Collaborate with multidisciplinary team members for coordinated and holistic client care.
• Participate in client case conferences, community outreach, and health promotion activities.
• Supervise therapy assistants.
• Participate in clinical student placement.
• Participate in internal training.
• Provide service to various settings (e.g. Home Rehab, CR, CH, NH) as deemed competent.
