Qualifications

• Min Diploma in Social Work, Psychology or Counselling is required.

Responsabilités

• Provide care coordination and service linkage to clients with or at risk of mental health issues and/or dementia, with appropriate resources and support including caregiver support.
• Plan and conduct outreach to the community and stakeholders including faith-based, corporate and community organisations to promote mental wellness and mental health awareness.
• Plan and conduct targeted mental health and dementia case finding/screening to promote early identification and timely support.
• Plan and organize structured preventive activities and/or programmes for the community, those at risk of mental health conditions and clients.
• Conduct regular visits to monitor the well-being of the client (CCMS/CREST).
• Support in initial assessment of walk-ins and referral cases, follow up with intervention plans for clients as per the prescribed care plan.
• Involve in the evaluation and documentation of the programme.
• Any other duties as assigned by reporting officer or Head of Community Social Services

Formation
