Responsibilities

• Provide end-to-end case management through assessment, care planning, intervention and evaluation.
• Deliver clinical interventions to support patients and families during transitions between healthcare institutions and home.
• Develop, conduct and facilitate group work programmes to build social support networks.
• Engage the community through outreach programmes, health education and preventive strategies.
• Participate in development and implementation of programmes to improve quality of care.
