Job summary

Allied Search Pte Ltd is seeking a Medical Social Worker to assess the psychological, emotional, social, and practical impact of illness on patients and families. You will provide counselling, crisis intervention, and help connect clients with community resources and healthcare services.

Collaborating with the care team, you will assist with discharge planning and maintain confidential records. A Degree or Postgraduate Diploma in Social Work or equivalent is required; shortlisted candidates

• Provide counselling, crisis intervention, and emotional support to patients and their families.
• Help patients and families to access and utilize community resources, such as financial assistance, housing, transportation, and healthcare services.
• Collaborate with other healthcare professionals to develop treatment plans that address the patient's holistic needs.
• Provide support and resources for caregivers, recognizing the impact of illness on families.
• Assist with discharge planning, ensuring a safe and smooth transition back home or to another care setting.
• Maintain accurate and confidential records of their interactions with patients and families.

Education
