My client is an established local provider of health and senior care services who is currently looking to fill a Medical Social Worker position. The role involves conducting psychosocial and financial assessments, providing casework and clinical interventions, coordinating care with residents and families, and collaborating closely with the multidisciplinary team to address residents’ medical, nursing, therapy and social needs.
Responsibilities
1. Admissions & Care Coordination
- Screen referrals and collaborate with relevant teams to assess residents’ suitability for admission.
- Coordinate and conduct onsite assessments when further evaluation is necessary.
- Provide supervision and support in completing pre-admission processes.
2. Casework & Clinical Interventions
- Support residents’ transition and adjustment to the nursing home environment and provide psychosocial interventions, counselling and emotional support to residents and/or their next-of-kin.
- Conduct biopsychosocial and financial assessments to develop comprehensive, person-centric care plans.
- Facilitate family visits and other engagements that promote family relationships, provide appropriate caregiver support and connect families with relevant community resources.
- Facilitate family conferences and discussions on care planning, including discharge planning, Advance Care Planning (ACP), Lasting Power of Attorney (LPA) and deputyship.
- Review supporting documentation for referrals to alternative care services and collaborate with the multidisciplinary team on assistive equipment and discharge planning, where required.
3. Financial Assistance & Subsidies
- Assess residents’ and families’ eligibility for subsidies and financial assistance.
- Review complex financial assistance cases and facilitate relevant discussions or meetings, including deviation, MediFund and MFEC cases.
- Support Case Management Executives with applications for care schemes.
4. Reporting, Administration & Team Management
- Ensure residents’ profiles, movements and relevant information are accurately maintained in the required databases.
- Review and update next-of-kin information and residents’ social reports.
- Liaise with Finance department on residents’ accounts receivable matters, where required.
- Supervise and support members of the case management team to ensure effective service delivery.
Requirements
- Degree or Postgraduate Diploma in Social Work.
- Registered with the Singapore Association of Social Workers (SASW).
- Prior experience in healthcare social work is preferred.
- Minimum 3 years of relevant experience, preferably in intermediate or long-term care settings.
- Strong interpersonal, communication and problem-solving skills.
- Compassionate, caring, positive and self-motivated, with strong professional discipline.
- Able to work effectively both independently and as part of a multidisciplinary team.
- Proficient in Google Workspace, including Gmail, Google Docs, Sheets and Slides.
