Job summary

TOUCH COMMUNITY SERVICES LIMITED is seeking a dedicated caregiver support to assist Occupational Therapists and Physiotherapists in delivering home therapy. You will help implement treatment plans, screen for risks, and coordinate referrals under therapists’ guidance.

The role emphasizes teamwork, timely documentation, infection control, and participation in training and quality initiatives. Based in Ang Mo Kio, it offers hands-on community care exposure.

Qualifications

• O level qualification with health care related training.
• 2–3 years of working experience in health care or community service.
• Proficient in MS Word, PowerPoint, Excel and Outlook.
• Familiar with social and health care resources.

Responsibilities

• Assist therapists in implementing treatment plans for routine cases.
• Screen for falls risk factors, caregiver burden and other needs.
• Refer clients to other care services under therapists' direction.
• Provide timely feedback to therapists in charge.
• Maintain timely documentation of observations during sessions.
• Participate in quality improvement projects and trainings.
• Attend department meetings and maintain inventory of equipment.
• Adhere to infection control procedures during home visits.
