Job summary

TOUCH COMMUNITY SERVICES LIMITED is seeking a dedicated caregiver support to assist Occupational Therapists and Physiotherapists in delivering home therapy. You will help implement treatment plans, screen for risks, and coordinate referrals under therapists’ guidance.

The role emphasizes teamwork, timely documentation, infection control, and participation in training and quality initiatives. Based in Ang Mo Kio, it offers hands-on community care exposure.

Qualifications

• O level qualification with health care related training.
• 2–3 years of working experience in health care or community service.

Responsibilities

• Assist therapists in implementing treatment plans for routine cases.

Job description

Job Summary:

The core function of this position is to support Occupational Therapist (OT) and Physiotherapist (PT) in provision of home therapy.

Principal Responsibilities and Duties:

• Assist therapists in implementing treatment plan for routine cases as directed by therapists.
• Support therapists to screen for falls risk factors, caregiver burden and other needs
• Support in the care of clients by referring them to other care services under the direction of therapists
• Provide timely feedback about client and/or caregiver to therapists in-charge
• Maintain timely and accurate documentation of observations made during sessions about client and/or caregiver
• Participate and support in quality improvement project, research or any new initiatives as assigned
• Participate in in-house trainings and department meetings
• Maintain inventory and equipment used for therapeutic activities during home therapy and at the centre
• Comply and support in infection control procedures during home visits
• Support in scheduling appointments for clients if required

Essential Skills and Qualifications:

• Min. O level qualification with health care related training.
• 2 to 3 years of working experience in the health care or community service sector
• Proficient in MS Word, PowerPoint, Excel and Outlook
• Familiar with social and health care resources
