Job summary

Ren Ci Hospital in Woodlands, Singapore, is seeking a dedicated nurse for home health care services. The role focuses on safe patient care, accurate documentation, and independent care planning in a community/home setting.

The ideal candidate holds a Diploma in Nursing with at least 2 years' community/home care experience, plus the ability to perform complex procedures and train caregivers. Palliative/oncology training is advantageous.

Qualifications

• Diploma in Nursing is required.
• 2 years of experience in community nursing/home care.
• Palliative care/oncology training is an added advantage.
• Capable of performing a wide range of complex nursing procedures without immediate on-site supervision.
• Ability to work independently with strong decision-making in a home setting.

• Initiating/reviewing nursing care plan based on assessment.
• Maintaining accurate nursing documentation.
• Performing nursing procedures such as wound dressing, urinary catheter insertion, nasogastric tube insertion, medication packing, etc.
• Regular evaluation of care provided for home care services.
• Provide caregiver training and education to client and families on home health care procedures and strategies.
• Reporting of any incidents to Nurse Lead.
• Participate in quality-improvement projects to enhance care and services.
• Identify service gaps and recommend improvements for home care service provision.
• Participate in integrated Home Health project to provide palliative care services including telesupport

Job Requirements
