Job description

Join in Community Care

Every day, clients with complex functional, medical, and end-of-life needs rely on our Home Personal Care (HPC) and Enhanced Home Personal Care (HPC+) services to live with dignity in the comfort of their own homes. As an Associate Nurse Manager, you will stand at the intersection of hands-on community nursing and team leadership.

In your first month, you will be on the ground-stepping directly into the care environment to assess complex cases, support family caregivers, and guide staff nurses through real-time clinical decisions. Working alongside the Nurse Manager, you will serve as both a clinical mentor and operational anchor, ensuring our home care delivery remains safe, ethical, and deeply person-centered.

Impact and Responsibilities
Clinical Leadership & Direct Care
  • Provide direct nursing care and clinical oversight for clients with complex health conditions, chronic disease burdens, and palliative care needs across the community.

  • Guide care planning, clinical risk assessments, and timely escalation protocols in full compliance with organizational standards and scope of practice.

  • Conduct regular clinical audits and on-site reviews to maintain quality, safety, and continuity of home-based care.

  • Maintain clear, accurate, and compliant clinical documentation across all client records and escalation notes.

Team Coaching & Supervision
  • Partner with the Nurse Manager to lead, mentor, and support nursing and care staff in their daily field practice.

  • Conduct on-the-ground coaching, competency assessments, and onboarding programs for new nursing and care team members.

  • Oversee daily workload balance, promote safety adherence, and manage clinical queries raised by front-line staff.

  • Identify operational risks or staffing challenges early and escalated them systematically to senior nursing management.

Multidisciplinary Collaboration & Service Integration
  • Work closely with doctors, allied health professionals, social workers, and community partners to deliver integrated care plans.

  • Active participation in case conferences and multidisciplinary reviews to optimize client outcomes and prevent avoidable hospital readmissions.

  • Facilitate smooth care transitions, step-down care pathways, and discharge planning between hospitals and community settings.

Quality, Safety & Governance
  • Support continuous quality improvement (QI) projects, clinical risk management, and incident investigation/follow-up.

  • Ensure all nursing practices uphold the ethical, professional, and legal standards established by the Singapore Nursing Board (SNB).

Qualifications and Skills Requirements
  • Professional Registration: Valid Registered Nurse (RN) license with the Singapore Nursing Board (SNB).

  • Education: Bachelor's Degree in Nursing or an equivalent qualification recognized by the SNB.

  • Experience: Minimum 4 to 6 years of post-registration nursing experience, with preference given to background in community nursing, eldercare, primary care, or home care environments.

  • Clinical Knowledge: Strong clinical grounding in chronic disease management, geriatric nursing, and community risk mitigation.

Preferred Qualifications
  • Prior experience as a Senior Staff Nurse, Nurse Clinician, or in a supervisory/team lead capacity.

  • Post-basic certificate or specialized training in Community Nursing, Gerontological Nursing, Palliative Care, or Healthcare Leadership.

Core Competencies
  • Proven ability to coach, guide, and support junior nurses and care staff in dynamic, field-based settings.

  • Sound clinical judgment with strong risk identification and timely escalation capabilities.

  • Clear communication and collaborative interpersonal skills when interacting with clients, family members, and multidisciplinary partners.

What We Offer
  • Meaningful Impact: Direct leadership role shaping home-based healthcare and enabling seniors to age in place.

  • Professional Development: Continuous clinical training, leadership development opportunities, and sponsorship for relevant post-basic certifications.

  • Supportive Environment: Collaborative culture focused on high clinical standards, peer mentorship, and team well-being.
