Job summary

Chen Su Lan Methodist Children’s Home is seeking an SSF-R Social Worker to provide intensive, home-based intervention to reunify CYPs with their families in a safe, timely and sustainable manner.

You will engage with CYPs, families and stakeholders, conduct needs and risk assessments, develop and monitor case plans, and document progress while coordinating with multi-disciplinary teams to support successful reunification.

Qualifications

• Requires a degree in Social Work and at least 2 years of related experience.
• Demonstrates knowledge in working with children, youth and families.
• Ability to communicate in English and local languages/dialects is an advantage.

Responsibilities

• Engage CYP and family in a therapeutic manner and conduct needs/risk assessments.
• Develop and follow up on case plans; draft social reports and maintain documentation.
• Collaborate with multidisciplinary teams to deliver reunification strategies and interventions.
• Provide home-based intervention and support to families and ensure timely step-down when appropriate.
• Attend supervision and professional development to enhance practice.
