Job summary

National Healthcare Group Polyclinics is seeking a Social Work Assistant to support medical social workers by screening referrals, collecting basic biodata and social information, and conducting preliminary intake assessments.

You will assist with simple casework and field visits, perform admin duties such as data entry and database upkeep, and participate in various projects to improve client services.

Qualifications

• Phone screening of new referrals to medical social workers to collect basic biodata and social information.
• Preliminary intake assessment on patients’ concerns.
• Support simple casework and field visits as assigned by MSWs; data entry and project participation.

Responsibilities

• Phone screening of new referrals to medical social workers to collect basic biodata and social information and to conduct preliminary intake assessment on patients’ concerns.
• Manage simple casework and field visits as assigned by the MSWs; perform administrative duties such as data base upkeep and participate in various projects.
