THYE HUA KWAN NURSING HOME LIMITED seeks a dedicated care assistant to support registered nurses in providing personalized, cost-effective care to residents at our Singapore facility. You will assist with basic nursing tasks, document care activities, and help maintain a safe, clean environment.

Under the supervision of the RN, you will monitor vital signs, assist with medication administration, help transfer and position non-ambulant patients, coordinate therapy sessions, and arrange transport

• Complies with approved standards of nursing practices.
• Assists the RN to supervise care staff and healthcare attendants.
• Provides basic nursing care to the patients.
• Updates on changes in patient's conditions and needs.
• Ensures care delivery and nursing interventions as outlined in the care plan.
• Carries out nursing rounds with the RN.
• Supervises care staff to ensure comfort and good personal hygiene of patients under his/her care.
• Superescare staff to check, monitor and record patient's vital signs.
• Maintains accurate documentation of care given.
• Assists RN in serving medications to patients and comply with safe medication practices.
• Assists and guides care staff in the transfer and positioning of non-ambulant patients.
• Supervises care staff in diet serving and feeding of patients that require help.
• Supervises care staff in ensuring comfort and good personal hygiene of patients under his/her care.
• Assists in the admission and orientation of new patients.
• Performs rehabilitation activities such as motion exercise and walking with patients.
• Organizes and sends patient for appropriate therapy as prescribed, example occupational therapy.
• Co-ordinates visits for medical check-ups and transport arrangement.
• Accompanies patient to the hospital/polyclinic for follow-up appointments.
• Initiates actions in emergency situations; escape and provides supports to RN promptly.
• Works with RN to ensure a safe and clean environment in the home.
• Participates in clinical service projects.
• Participates in infection control campaign activities.
• Assumes additional responsibilities as assigned by RN, Nurse Manager.
• Participates in training and development of care staff.

JOB PURPOSE

The jobholder is responsible to provide personalized, and cost-effectivecareto the patients in the nursing home in accordance to established policies and procedures, standards of nursing care and practices.

DUTIES AND RESPONSIBILITIES

Primary Responsibilities and Duties (80%)

• 2.1.1 Complies with approved standards of nursing practices.
