Job summary

SASCO SENIOR CITIZENS' HOME in Singapore seeks an Administration and Social Work Support role to coordinate client referrals, admissions, and community links. You will assist with social support needs, maintain records, and liaise with clients, caregivers, rehabilitation teams, and external agencies.

The ideal candidate has a Diploma or GCE O-Levels with relevant social work experience, strong interpersonal skills, and familiarity with ILTC systems such as NMTS and BRIGHT.

Qualifications

• Diploma or GCE O-Levels with relevant social work experience.
• Proficiency in office software and strong interpersonal communication.
• Familiarity with eldercare, ILTC systems, NMTS and BRIGHT system is an advantage.

Responsibilities

• Coordinate client referrals, intake, and admission processes.
• Assess clients' social and support needs and facilitate appropriate services.
• Link clients to community resources, special interest groups, and social activities.
• Support clients in enhancing social participation and quality of life.
• Coordinate and follow up on referrals to social, healthcare, and community services.
