Job summary

NTUC Health Co-operative Limited is seeking a Programme Associate for our Fun with Friends Programme on a 2-year contract. You will join a dedicated care team delivering person-centred care to seniors, including persons with dementia, through multi-modality group activities.

With support from our professionals, you will grow in a nurturing, collaborative environment, facilitating sessions, assisting with planning and documentation, and communicating with families.

Qualifications

• Nitec / Diploma / Certificate in Healthcare or equivalent course.
• Some experience in social service or the healthcare industry would be an advantage.
• Facilitation skills and healthcare knowledge would be an advantage.

Responsibilities

• Facilitate various multi-modality group activities for persons with dementia as assigned by the programme executive; including exercise sessions, meal preparation groups, cognitive games, music and art-related activities.
• Assist in activity planning and preparation to ensure the smooth execution of the group activities
• Manage clients with different mental, and physical conditions and behaviour of concerns relating to dementia and assist in the monitoring of client’s health status
• Assist in client’s care including providing necessary assistance for transfers, toileting and functional mobility
• Assist in administering assessments to clients and caregivers, perform data collection
• Perform timely documentation for the client post-session
• Assist to provide timely updates to family/NOK of client's engagement
• Support staff or volunteers relating to assist with running the programme
• Assist in inducting new community care associates and provide necessary guidance
• Assist in the procurement of activities materials, with approval sought from RO/Programme Executive
• Perform general housekeeping to the programme premise
