Job summary

Tzu-Chi Foundation (Singapore) seeks a dedicated Social Worker to provide psycho-social care for patients and families, and to lead ACP discussions and care planning. You will also support bereavement services and coordinate assistance and transport for those in need.

Ideal candidates hold a degree or postgraduate diploma in Social Work with a genuine interest in elderly care, strong communication, and teamwork abilities.

Qualifications

• Degree or postgraduate diploma in Social Work.
• Preferably 2 years relevant experience.
• Interest to work with elderly.
• Displays compassion, a positive and caring attitude towards patients, internal and external clients.
• Excellent communication, interpersonal and problem solving skills.
• Meticulous, diligent and a good team player.
• Adaptable and comfortable with changes.

Responsibilities

• Provide psycho-social/emotional care and support to patients and families.
• Lead in ACP discussions and completion. Complete Preferred Care Plan.
• Provide Bereavement Support.
• Coordinate in-house Financial Assistance for needy patients.
• Coordinate Medical Escort & Transport for patients.
• Coordinate in referring patients for Respite Care, Nursing Home and other necessities.
