Job summary

VANGUARD HEALTHCARE PTE. LTD. is seeking a Community Care Associate (Home Care) to support the Senior/Staff Nurse in delivering quality home-based healthcare services.

The role focuses on providing care to clients at home in Pasir Ris West Region, including night shifts and weekend rosters. Responsibilities include maintaining a safe home environment, assisting with daily living tasks, monitoring vital signs, and coordinating with therapists for at-home activities.

Qualifications

• GCE N level or higher with healthcare exposure.
• Training in healthcare support is preferred.
• At least 1 year of relevant work experience.

Responsibilities

• Maintain a safe and clean home environment for clients, including light housekeeping when needed.
• Attend to basic nursing and personal care needs (feeding, toileting, etc.).
• Ensure client comfort and hygiene (bed/shower baths).
• Monitor vital signs (temperature, pulse, respirations, blood pressure).
• Collaborate with therapists to implement home-based activities and exercises.
• Perform other duties as assigned by the supervisor.

Job description

The Community Care Associate (Home Care) will assist the Senior / Staff Nurse in the delivery of required and optimal level (quality care) of healthcare services, based on the defined care model, to clients in the home care setting.

Work Location: Pasir Ris West Region

Working Hours: Required to work night hours, roster basis including weekends

• Maintains a safe and clean home environment for clients, which may include housekeeping, if the client and/or client’s caregiver is unable to do so.
• Attend to basic nursing and personal care needs of clients, e.g., feeding, toileting, etc.
• Ensures the comfort and good personal hygiene of the client, e.g. bed bath / shower bath etc.
• Checks, monitors, and records clients’ vital signs such as temperature, pulse, respirations and blood pressure.
• Work with the therapists on implementation of activities and exercises for clients at home.
• Any other duties as assigned by the supervisor.

Requirements

• GCE N level
• Preferably attained training in either of the following courses:

1. WSQ Higher Cert in Healthcare Support (Nursing Care)
2. ITE Approved Training Centre Healthcare (Home Care) program

• Good communication and interpersonal skills. Ability to converse in major dialects is an advantage.
• Strong organisation abilities.
• At least 1 year of experience.
