Job summary

GEYLANG EAST HOME FOR THE AGED in Singapore is seeking a compassionate Social Worker to support elderly residents, coordinate admissions, and provide psychosocial care.

The role requires a degree in Social Work with 1–2 years’ experience; fresh graduates welcome. You will liaise with families, government agencies, and community partners to coordinate care and NMTS renewals, while contributing to resident-focused programmes. Weekend availability is required.

Qualifications

• Minimum degree in Social Work.
• 1–2 years of experience in the Social Service Sector preferred. (Fresh graduates with a passion for the sector are welcome to apply)

Responsibilities

• Counselling for new referrals, assessing financial, health, and social circumstances.
• Coordinate and support resident admissions, orientations, and discharges.
• Manage onboarding, including appointment scheduling, subsidy applications, and enquiries.
• Frontline counter duties on a rotational basis.
• Build rapport and provide emotional and psychosocial support to residents.
• Assist in development and implementation of individualised care plans.
• Monitor and renew NMTS for elderly residents.
• Liaise with families, government agencies, and community partners to coordinate care and services.
• Maintain and submit accurate documentation and reports to authorities.
• Contribute to resident-focused programmes and engagement projects.
• Respond to ad hoc crisis situations with timely support.
• Provide administrative, operational, and logistical support for client services and outreach.
• Available to work two weekends per month and on major public holidays.

• Respond effectively to ad hoc crisis situations, offering timely support and intervention.

Requirements
