Job summary

Allied Search Pte. Ltd. is seeking a Care Concierge in Singapore to act as the primary contact for families of nursing home residents.

The role involves guiding caregivers and residents through the pre-admission process, explaining fees and care philosophy, and welcoming new residents on admission day. The position requires interactions with a multidisciplinary care team, ongoing engagement with residents and families, and participation in care planning discussions to capture social background

Qualifications

• Must possess a Diploma or Degree with at least 2 years of work experience.
• Strong interpersonal and communication skills are essential.
• Ability to engage with families, residents, and the care team professionally.

Responsibilities

• Serve as the main liaison for next-of-kin of nursing home residents.
• Engage caregivers and residents pre-admission to address concerns and needs.
• Conduct pre-admission counselling covering fees, services, philosophy of care, living conditions, and expectations.
• Welcome residents on admission day, conduct orientation, and help settle into the environment.
• Maintain ongoing engagement with residents and caregivers with the multidisciplinary care team.
• Participate in care planning discussions by providing insights on residents' social background and preferences.
• Develop and maintain touchpoints throughout the resident journey from pre-admission to discharge.
• Assist in measuring customer satisfaction through focus groups, audits, and surveys.
• Support implementation of service recognition programs and events such as the Service Excellence Award.
• Provide feedback on service gaps and propose improvement initiatives and training.

As a Care Concierge, Service Quality (Community Settings), you will be responsible for the following duties:

• Serve as the main liaison and single point of contact (Care Concierge) for next-of-kin of nursing home residents.
• Engage caregivers and residents pre-admission to address concerns and understand specific needs.
• Conduct pre-admission counselling covering fees, services, philosophy of care, living conditions, and expectations.
• Welcome residents on admission day, conduct orientation, and ensure smooth settling into the new environment.
• Maintain active engagement with residents and caregivers throughout their stay, addressing concerns collaboratively with the multidisciplinary care team.
• Participate in multidisciplinary care planning discussions by providing insights on residents' social background, preferences, and habits.
• Develop and maintain key touchpoints throughout the resident journey from pre-admission to discharge.
• Assist in measuring and monitoring customer satisfaction through focus groups, audits, and surveys.
• Support implementation of service recognition programs and events, such as the annual Service Excellence Award.
• Provide feedback on service gaps during team meetings and propose initiatives and training programs for continuous improvement.
