Job summary
Recruit Express Pte Ltd. is seeking an ACP Coordinator to facilitate Advance Care Planning discussions for targeted hospital patients.
This role requires a Bachelor's degree in Nursing, Psychology, Social Work, Counselling, or related Allied Health field and offers an immediate start through 31 March 2027. You will advocate ACP to patients, families, and healthcare professionals, ensuring documentation is accessible and up-to-date, and supporting KPIs, events, and quality improvement projects
Qualifications
- Bachelor’s degree in Nursing, Psychology, Social Work, Counselling or related Allied Health field.
- Strong interpersonal, communication and multi‑tasking abilities.
- Proficient with Microsoft Office (Word, Excel, PowerPoint).
- Experience in Psychology/Counselling or related field is a plus.
Responsibilities
- Facilitate ACP discussions and complete advance care plans
- Ensure patients and families have access to ACP documentation
- Provide timely access to ACP documentation for care providers
- Review and update ACP with patients or nominated spokespeople
- Contribute to service improvement and data submission for ACP KPIs
- Support ACP events, trainings and community awareness activities
