Job summary

Fakeeh Care Group in Jeddah is seeking a compassionate Social Worker to provide direct and indirect contact with patients, ensuring a supportive experience during visits or stays. You will coordinate care, assist with discharge planning, and address social concerns with multidisciplinary teams.

Fluent in English and Arabic, with 0–2 years of experience and a Bachelor’s in sociology, psychology, or a related field.

Qualifications

• Bachelor's degree required in sociology, psychology, or a related field.
• Strong communication and interpersonal abilities.
• Ability to adapt in challenging environments.
• Proficiency in MS Office and data documentation.

Responsibilities

• Provide direct and indirect contact with patients to support service delivery.
• Coordinate care management and discharge planning in collaboration with multidisciplinary teams.
• Prepare and obtain medical reports from the Medical Records Department.
• Conduct psychosocial assessments and counseling for patients and families.
• Support patients and families during stressful or end-of-life situations.
• Assist in arranging interpreters and clergy when needed.
• Maintain patient information in the EMR and contribute to KPI reports when applicable.
