Our innovative Rapid Response & Intermediate Care service is expanding across Buckinghamshire - and we need you to join our Marlow team.

  • Your mission: To provide expert interventions and rehabilitation as part of a dynamic, tight-knit team that reacts quickly when people are in crisis.
  • Your aim: To help patients stay in their own home, empowering recovery and preventing unnecessary hospitalisation. You'll also support early discharge from hospital and patients at the end of life.
  • Your role: Add your expertise to our multidisciplinary community team working with Physiotherapists, Occupational Therapists, Nurses, Specialist Paramedics, Paramedics, Pharmacists, Community Healthcare Assistant Practitioners, or Healthcare Assistants.

Main duties of the job

#MissionRRIC

  • Providing triage, specialist assessment, review, case management, and evaluation of patients and carers' needs.
  • Planning care programmes to promote health, independence, and actively managing patient journeys.
  • Allocating workloads within the RRIC team.
  • Collaborating with professionals to ensure timely and appropriate care delivery.
  • Demonstrating professional skills in leadership, care planning, coordination, and clinical care. Supporting the development of clinical pathways.

Job responsibilities

Person Specification

Education, Qualifications & Training

  • Registered Allied Healthcare Professional with HCPC - Physiotherapist.
  • Degree/Diploma in Physiotherapy or Occupational Therapy.
  • Recent CPD and relevant postgraduate training.

Experience

  • Post or preregistration placements in relevant clinical areas.
  • Experience working in multidisciplinary teams and community/in-patient settings for at least 2 years post-registration.
  • Experience with electronic clinical systems, research, audit, staff management, and assessment processes.

Skills, Abilities & Knowledge

  • Clinical assessment, planning, and evaluation skills.
  • Excellent communication, case management, partnership development, and legal knowledge.
  • Report writing, IT skills, and teaching abilities.

Other Requirements

  • Valid driving license and access to a vehicle for travel across Buckinghamshire.

Salary: £37,338 to £44,962 per annum, pro rata.
