Under the direct supervision of the PACE Supervisor, the PCA II will perform and assist with personal care activities of daily living (ADLs) / instrumental activities of daily living (IADLs) for participants in the PACE program across all platforms of care delivery, including center-based, in-home, or virtual settings. The PCA II must be capable of performing the duties of a PCA I if assigned. Additionally, the PCA II supports the PCA III in gathering and reporting information related to care coordination, participant engagement, functional maintenance or improvement, fostering independence, enhancing socialization, and overall satisfaction. The PCA II communicates and collaborates with the Interdisciplinary Team (IDT) members and other PACE staff to align with the care plan, focusing on participant needs such as supplies, tools, health status, and physical and mental well-being.
