On behalf of the West Toronto Ontario Health Team (WTOHT), LAMP Community Health Centre is seeking a Project Manager for the LDPHM initiative. LAMP provides a wide range of programs to improve health and well-being, connecting people to medical services and community programs.
The WTOHT aims to improve healthcare experiences and outcomes in West Toronto by bringing together patients, caregivers, and providers to create a connected health and social care system.
Job Summary
The Project Manager will oversee the implementation of LDPHM, collaborating with partners, community organizations, and grassroots groups to promote preventative care, health promotion, and primary healthcare access. The role involves leading interdisciplinary teams and ensuring service delivery aligns with project goals and funder requirements.
Primary Responsibilities
- Lead and manage an interdisciplinary team, ensuring alignment with organizational values and strategic goals.
- Oversee LDPHM implementation within the WTOHT catchment area.
- Promote community engagement, health literacy, and service access.
- Support increased attachment to primary care and participation in screenings.
- Monitor programs, identify risks, and implement improvements.
- Foster partnerships and coordinate stakeholder engagement.
- Manage data collection, monitoring, evaluation, and reporting.
- Organize outreach, community events, and promotional activities.
- Contribute to health messaging via social media and other platforms.
- Adapt project scope responsively to community needs.
- Perform additional duties as needed.
Required Qualifications and Skills
- University Degree in Health Sciences, Management, or equivalent experience (Master’s preferred)
- 3-5 years project management experience in healthcare or community settings
- Regulated Health Professional designation is an asset
- Experience working independently and in partnership management
- Understanding of vaccine hesitancy and barriers to healthcare access
- Knowledge of health promotion, especially in diverse and marginalized communities
- Experience collaborating with community organizations and residents
- Event and program planning experience
- Knowledge of Ontario healthcare system is an asset
- Sensitivity to diverse cultural and economic backgrounds
- Excellent communication skills
- Commitment to anti-oppression principles and equity
- Strong organizational and problem-solving skills
- Ability to travel within GTA and work flexible hours including evenings and weekends
Proficiency in Computer Skills
- Ability to work independently and remotely
- Strong time management and self-direction skills
