Job summary

Quantum Health, Inc. is seeking a Transplant Nurse (PCG) to coordinate care for members with transplant potential. The role blends clinical nursing, care management, and member advocacy in a hybrid/remote setting from Dublin, OH.

You will identify candidates, apply nursing processes, document activities, and collaborate with care teams to optimize outcomes while promoting the company’s mission and values.

Qualifications

• Active, unrestricted RN license in good standing; multi-state license may be required.
• CCM credential strongly preferred on hire or within 24 months.
• Bachelor’s degree in Nursing preferred.
• Minimum two years clinical experience with direct patient care.
• Excellent computer skills including Microsoft applications.
• Excellent verbal and written communication skills; time management and autonomy.
• Solid organ or bone marrow transplant experience desired.

Responsibilities

• Identify and assess transplant candidates, including solid organ and bone marrow transplantation.
• Apply nursing process to actively case manage transplant members.
• Document activities in appropriate database; maintain care coordination.
• Collaborate with health care teams; provide updates to client executives and employer contacts.
• Educate members on transplant process and benefit information; advocate for members.
