Job summary

Transcarent is hiring a Registered Nurse to serve as a trusted resource, delivering triage, healthcare navigation, and advocacy across a diverse member population.

You will assess health concerns, prioritize needs, and recommend evidence-based care while supporting oncology patients and promoting whole-person health in a hybrid office setting. A BSN and current RN license are required, with at least 3 years of direct patient care experience.

Qualifications

• Bachelor's degree in nursing (BSN) or equivalent.
• Active, unrestricted RN license; Compact (NLC) license.
• Minimum of 3 years of recent clinical experience in direct patient care.
• Strong clinical acumen, critical thinking, and communication skills.
• Technological fluency with multiple systems.

Responsibilities

• Empower Members: Educate and guide members to appropriate clinical programs and services.
• Oncology Support: Manage oncology-related needs across diagnosis, treatment, survivorship, and supportive care.
• Conduct Assessments: Perform telephonic, holistic clinical assessments to identify needs and priorities.
• Support Whole-Person Health: Address medical and behavioral health needs across the care continuum.
• Educate and Refer: Provide evidence-based education and refer to internal programs and trusted partnerships.
