AFM Family & Internal Medicine in Fairfax, VA is seeking a Psychiatric Mental Health Nurse Practitioner to lead behavioral health care within our primary care practice, working with physicians and care management to bring psychiatric care to patients who may wait months for referrals.

The role emphasizes integrated, team-based mental health care in a supportive setting, with telehealth and EHR experience valued. Remote position with collaboration across our care team.

Formación

• Master’s or doctoral PMHNP degree from an accredited program.
• Current PMHNP-BC certification through ANCC.
• Active, unrestricted Virginia NP license with prescriptive authority or eligible.
• Active DEA registration or willingness to obtain one.
• Psychiatric experience preferred; primary care/integrated behavioral health is a plus.
• Comfortable with telehealth and EHR systems.
• Strong communication skills and a patient-first approach.
• Ability to work well in a fast-paced, collaborative environment.

Responsabilidades

• Provide psychiatric evaluations, diagnoses and treatment plans for adult patients.
• Prescribe and manage psychiatric medications, including monitoring and follow-up.
• Serve as the psychiatric consultant for our CoCM and behavioral health integration program.
• Manage conditions seen in primary care, including depression, anxiety, ADHD, bipolar disorder, PTSD and substance use concerns.
• Offer brief psychotherapy and supportive counseling as appropriate.
• Document encounters accurately and on time in our EHR.
• Collaborate with physicians, nurses and support staff to keep care coordinated.
• Follow up on labs, referrals and patient questions promptly.
• Follow all clinical protocols, HIPAA regulations and safety standards.
