Job summary

HealthSnap is seeking a Remote LPN for the Care Navigator role to support patients in chronic care management. The position requires a valid Illinois LPN license and 3+ years of relevant experience. You will provide education and empower patients to manage their chronic illnesses effectively. Responsibilities include documentation, problem-solving, and communication with patients. Qualifications

• 3+ years of experience in primary care practice, cardiology, internal medicine, home care, or chronic care management.
• Ability to work independently and as part of a team.
• Reliable internet connection and HIPAA-compliant work area.

Responsibilities

• Complete phone consultations with patients providing support and education about chronic conditions.
• Educate patients about their health conditions and empower them to manage chronic illnesses.
• Maintain accurate patient records and document interactions according to protocol.
