This role emphasizes person-centered care, ongoing documentation, family communication, and supervision of caregivers. A current LPN/LVN license and CPR certification are required; orientation includes dementia

Qualifications

• Active LPN/LVN license in the state of practice.
• CPR certification required.
• Experience in long-term care, skilled nursing, rehab, memory care, or geriatric nursing preferred.
• Willing to complete Trilogy Legacy Dementia Education Program during orientation and annually thereafter.

Responsibilities

• Provide person-centered dementia care to support dignity, comfort, independence, and quality of life.
• Deliver nursing care per individualized care plans and physician orders.
• Conduct nursing assessments and communicate changes to families and care team.
• Administer medications and treatments safely; monitor responses and document accurately.
• Develop and update individualized care plans based on resident needs and health outcomes.
• Collaborate with families, physicians, therapists, and team members for coordinated care.
• Provide guidance and supervision to nursing assistants and caregivers to ensure excellence in dementia care.
