The state of Florida Clinical Provider (NP/IBCLC) is responsible for ensuring direct patient care is evidence-based and aligns with SimpliFed's maternal health program requirements. Providers will conduct lactation-related clinical assessments, oversee remote patient monitoring, administer CPOE orders within scope of practice, and screen for maternal mental health.
ROLE:
The role of the Clinical Provider (NP/IBCLC) is to uphold the highest standards of care for the company and its patients. Providers will engage in telehealth, virtual video, and face-to-face appointments to provide care and assist patients with personal goals and those goals prescribed by their health care team. Providers are responsible for all clinical decision-making in alignment with SimpliFed\'s clinical protocols associated with the provision of Professional Services.
VENUE:
This role works full time remotely using SimpliFed approved devices.
POSITION SUMMARY:
The SimpliFed Clinical Provider (NP/IBCLC) will conduct professional services consultations with patients via telehealth. Providers will assess patients based on their professional field, provide education on topics such as lactation, infant feeding, hypertension, gestational diabetes, anemia, thyroid levels, and maternal mental health. Providers will document findings in the exam form, provide resources to patients, schedule follow-up appointments as needed, and make referrals to outside and referring providers when appropriate.
REQUIREMENTS:
- Master’s or Doctoral degree in the respective clinical field (Nurse Practitioner)
- Professional licensure as a Nurse Practitioner, in good standing with the governing board.
- Professional certification as an International Board Certified Lactation Consultant (IBCLC) or Certified Lactation Counselor (CLC), in good standing with the governing board.
- 3 years of direct clinical experience in a hospital, clinic, or other healthcare setting with a minimum of 2 years as a CLC/IBCLC.
- Interest in maternal health and improving outcomes.
- Ability to provide education and clinical support to empower families in infant feeding and care decisions.
- Ability to work independently and make decisions aligned with SimpliFed policies.
- Strong active listening skills and sound judgment.
- Demonstrated written and oral communication skills, and strong charting abilities.
- Knowledge of clinical workflows, health care quality and compliance standards, EHR data management, clinical documentation, HIPAA, and related regulations.
- Ability to accept constructive feedback and improve service to SimpliFed patients, partners, and mission.
POSITION SPECIFICS:
- Providers will see patients for maternal health and perinatal matters. Services include, but are not limited to:
- Lactation and Infant Feeding services provided by IBCLC/CLC credentialed professionals.
- Remote Patient Monitoring for Blood Pressure and Gestational Diabetes for qualified providers.
- Perinatal Mental Health services provided by all SimpliFed providers and Credentialed Perinatal Mental Health Counselors (PMHC).
- Other maternal health issues such as nutrition, anemia, thyroid issues.
- Providers will communicate with patients via the company\'s electronic medical record system and virtual platform.
- Communications should focus on clinical relevance and empower patient health literacy and evidence-based decisions.
- Review intake forms, obtain history, and perform virtual physical evaluation of pregnant or postpartum patients.
- Conduct Perinatal Mental Health Assessment using standardized instruments and tools provided in Exam Forms, plus general observation of mental state.
- Provide education on topic(s) related to the appointment type.
- Obtain relevant medical history and complete Exam Forms in full, including times, history, intake, baby wellness, diagnoses, and procedure codes.
- Document all events, observations, evaluations, assessments, and instructions on the same day as the appointment.
- Develop an individual Care Plan for each patient with written instructions, resources, and follow-up plan.
- Complete referrals/coordination of care as needed and send to the proper entity.
- Schedule subsequent appointments with patients prior to completing the current appointment.
