Job summary

iCareManager (iBilities LLC) in Lahore, Pakistan seeks a senior software engineer to take over the emrgen.ai codebase, infrastructure, and AI components from the external vendor, complete handover with documentation, and make it ours.

In the first months you will own the code, infrastructure, models and prompts, finish the vendor handover, then decide where the product goes next and build it, working with our Chief Product Officer and the Pakistan engineering team.

Qualifications

• 8+ years of professional software engineering experience.
• 3+ years shipping AI features into production.
• Experience with an EHR/EMR system for US healthcare customers while based in Pakistan.
• Hands-on work with large language models: prompts, retrieval, evaluation, cost/speed control.
• Experience inheriting a system and making it maintainable.
• Knowledge of HIPAA, audit trails, and handling PHI in AI pipelines.
• Clear understanding of clinical documentation workflows and clinician trust.
• Strong written English for customer-facing documentation.

Responsibilities

• Take over the emrgen.ai codebase, cloud environment, and AI components from the external vendor with full documentation and access.
• Audit current architecture, security, data handling, and model behavior; deliver a plan for fixes or rebuilds.
• Run AI documentation features end-to-end from clinician input to signed, structured notes.
• Measure AI quality with evaluation sets and track accuracy and editor needs by clinicians.
• Build guardrails to ensure clinical facts come from source and require clinician approval before entering records.
• Enforce access controls, audit logging, and data-sharing rules with outside model providers.
• Connect emrgen.ai to the iCareManager platform and Medicaid billing requirements to turn documentation into billable services.
• Set up releases, monitoring, and on-call coverage to preempt production problems.
• Mentor engineers on the Pakistan team and assist with hiring.

We have been live since 2011 and serve hundreds of provider organizations across 30-plus states. Our modules include eMAR, CareTracker, ISP and PCP planning, incident reporting, billing, fleet management, and nursing assessments.

We also build a family of AI products, including eMAR.ai, emrgen.ai, iddbilling.ai, and LIFEPlan.ai. Billing sits at the center of that work, because it is where provider organizations either get paid or do not.

iCareManager is a HIPAA-compliant, SOC 2 Type II certified web-based EHR and management platform for IDD, assisted living, ICF, and human services providers. We have been live since 2011 and serve hundreds of provider organizations across 30-plus states.

We also build a family of AI products. emrgen.ai is our clinical documentation and EMR product for IDD providers, Article 16 clinics, and behavioral health professionals. It combines EHR, EMR, and AI-powered documentation, and it is built to be Medicaid ready and OPWDD compliant.

ROLE SUMMARY

emrgen.ai was built with an outside engineering vendor. It is live, and we are now bringing it fully in-house. We need one senior engineer who takes it over, understands every part of it, and makes it ours.

In your first months you will take ownership of the code, the infrastructure, the models, and the prompts, and finish the handover with the vendor. After that you will decide where the product goes next and build it. You will work directly with our Chief Product Officer and our engineering team in Pakistan.

WHAT YOU WILL OWN

• Take over the emrgen.ai codebase, cloud environment, and AI components from the outside vendor, and finish the handover with full documentation, clean access, and no single point of failure.
• Audit what is running today, covering architecture, security, data handling, and model behavior, and deliver a written plan for what to keep, fix, or rebuild.
• Run the AI documentation features end to end, from clinician input to the structured note a clinician reviews and signs.
• Measure AI quality with real evaluation sets, and track accuracy, invented content, and how often clinicians have to edit a generated note.
• Build guardrails so the AI never adds clinical facts that were not in the source, and so a clinician approves every note before it enters the record.
• Protect patient data by enforcing access controls, audit logging, and clear rules on what may reach any outside model provider.
• Connect emrgen.ai to the rest of the iCareManager platform and to state Medicaid billing requirements, so documentation turns into billable services.
• Set up releases, monitoring, and on-call coverage so we find production problems before our customers do.
• Mentor engineers on the Pakistan team and help us hire the people who join you.

WHAT YOU BRING

• 8 or more years as a professional software engineer, including time as a technical lead.
• 3 or more years shipping AI features into production, not demos.
• You have built or heavily worked on an EHR or EMR system for United States healthcare customers while based in Pakistan, and you can name the system and your part in it.
• Hands-on work with large language models, including prompt design, retrieval, evaluation, and controlling cost and speed. Speech-to-text experience is a strong plus.
• Real experience inheriting a system someone else built and making it maintainable.
• Working knowledge of HIPAA, audit trails, and handling protected health information inside AI pipelines.
• A clear understanding of clinical documentation workflows, and of what a clinician will and will not trust.
• Written English strong enough for customer-facing documentation, and comfortable working with United States hours overlap.

NICE TO HAVE

• Experience in IDD, behavioral health, or Medicaid billing.
• Familiarity with HL7 or FHIR.
• Experience supporting a SOC 2 audit.
• Experience with machine learning operations, such as model versioning and monitoring.
