Job summary

Indus Hospital is seeking a Pharm-D qualified pharmacist to ensure rational drug use, pharmacovigilance, and patient counseling across OPD, ER, inpatient, and oncology units.

The role includes coordinating with physicians, maintaining inventory, and ensuring regulatory compliance and staff training. The candidate should have registration and license validity and strong attention to drug interactions and delivery timelines.

Qualifications

Responsibilities

Job Description

TASKS & ACCOUNTABILITIES :

• To ensure proper handing over and taking over with writing detail note in over register as per protocols.
• To make sure timely dispensing and distribution of medication, ensuring patients’ profiles and CPOE for rational drug use, to prevent drug interactions and to promote rational drug use.
• To contact the physician for in-accuracy of medication order and document intervention (accepted or rejected) accordingly (if intervention is rejected for some reason will shared with clinical team for to present with formulary committee.
• In case of non-availability of medicine contact physician for switching to available alternate/substitute. If not then process for Non formulary arrangement through NFI policy.
• Ensure all orders are timely processed and prompt delivery of medications to the Nursing unit.
• Monitor and manage inventory stock levels, manage when needed.
• Prepare and forward departmental requisitions to warehouse.
• Receive supplies from warehouse, physically check batch, expiry, quantity, packaging, cold chain if applicable and shelving the stock in proper location at specified temperature.
• Make sure light sensitive, cytotoxic drugs, High alert and Look alike and sound alike drugs are placed accordingly.
• When duty assign at OPD pharmacy where medicines are dispense directly to patient, pharmacist must counsel the patient and respond to patients quires.
• When duty assigned at oncology / cytotoxic stations pharmacist will do the following:
  I. Making patient protocols: Check patient profiles, baseline labs of patient like CBC, LFT, and Creatinine before making chemotherapy protocol & preparation of regimen, three copies of protocols must be generated—one sent to nursing, one to patient, and one for Pharmacy record. II. Double check patient protocol and counter sign. III. Before issuance and preparation of drugs protocol prepared, CPOE order double check should be done by pharmacist also at chemo processing station. IV. Intrathecal drugs like Methotrexate, Cytarabine, & Hydrocortisone are issues in early morning (before dispensing regular chemo) to avoid errors or by specified by the policy and protocol. V. Patient profile review to make sure for right diagnosis, age and protocol/phase. VI. Once chemo regimen prepared pharmacist will counter check / recheck each drug and counter sign. VII. Hand over chemo of all patient to nursing for administration, for any reason drug is not administered/refused is also documented in excel sheet for record as per protocol.
• When duty assigned at ER Pharmacy, pharmacist manages to visit emergency room (bed side) for direct patient care and drug utilization review.
• While working at Inpatient pharmacy, pharmacist has to make sure all SOPs and protocol of sterile area are being followed.
• Make sure temperature/ humidity / pressure differentials in sterile area is being maintained and documented.
• Provide drug information to all health care professionals when required, and document it.
• Provide monthly reports to Pharmacy management all stock, detailing fast moving / slow moving / short expiry / expired / broken, damaged items.
• Conduct inspection of nursing units (like Wards, ICU, CCU, ER etc.) each month and monitor the proper storage and distribution of capital floor stock (crash carts), controlled drugs and emergency medications, and submit report to Deputy/Manager Pharmacy services and submit a copy to nursing management.
• Provide technical and administrative support, and conduct relevant trainings for staff.
• Prepare and present CMEs on various topics.
• Submit monthly report interventions, drug information to Deputy/Manager Pharmacy.
• Ensure temperatures / dusting/ refrigerator cleaning etc. are monitored and all related documentation is accurately maintained.
• Make sure planned preventive measure (PPM) is duly done of pharmaceutical refrigerators and ACs and other equipment of respective locations.
• Provide management with regular updates and monthly KPI reports.
• Providing training to interns, pharmacy resident.
• To fill PKPs and stock cover sheets in each shift.
• Any other task assigned by shift incharge / Pharmacy Management.
