Qualifications

Responsibilities

• Conduct Depression, Cognitive Impairment, and Medication Management assessments.
• Review patient profiles and document findings based on clinical criteria.
• Communicate with patients via outbound/inbound calls to explain care plans.
• Develop and maintain Chronic Care Management plans.
• Coordinate appointment scheduling and billing in collaboration with providers.
