Qualifications

• Must hold active USRN license in NY and BSN with PHRN license.
• Minimum of two years clinical or hospital experience.
• Strong critical thinking and independent decision-making skills.
• Excellent written and verbal communication, including phone/e-mail.
• Proficient with MS Office and EMR systems, preferably Epic.
• Amenable to night shifts.

Responsibilities

• Provide remote clinical care and assist patients with prescription requests.
• Visit follow-ups and manage non-urgent medical concerns and test result inquiries.
• Triage in-basket messages to ensure timely responses.
• Contact patients by phone for urgent matters per guidelines.
• Document encounters in Epic MyChart and Epic EHR accurately.
• Coordinate with local site teams via phone or email.
• Educate patients on chronic conditions such as diabetes, hypertension, asthma, COPD, and heart failure.
