Job summary

Qualifications

• BS Pharmacy degree (or equivalent)

Responsibilities

About the role

This is a Pharmacist position focused on patient medication management and clinical pharmacy services. The role involves assessing patient health problems, evaluating medication appropriateness and effectiveness, and collaborating with healthcare providers to optimize patient outcomes.

Key responsibilities

• Assess the status of the patient's health problems and determine whether prescribed medications are optimally meeting the patient's needs and goals of care

• Evaluate the appropriateness and effectiveness of the patient's medications

• Recognize untreated health problems that could be improved or resolved with appropriate medication therapy

• Follow the patient's progress to determine the effects of medications on health

• Consult with physicians and other healthcare providers in selecting medication therapy that best meets patient needs

• Advise patients on how to best take their medications

• Support the healthcare team's efforts to educate patients on important health steps such as exercise, diet, and preventive measures like immunization

• Refer patients to physicians or other health professionals to address specific health, wellness, or social services concerns

• Provide a consistent process of patient care that ensures appropriateness, effectiveness, and safety of medication use

• Apply specialized knowledge of the scientific and clinical use of medications in collaboration with other healthcare team members
