Job summary

North Bay Regional Center in California is seeking a Social Services Case Manager (Service Coordinator) to manage the delivery of services to NBRC clients, including those with intensive physical or emotional needs, ensuring that the client’s Individual Program Plan and its objectives are met.

Our Social Services Case Managers use person-centered planning to help clients access public and private resources before using regional center funds to purchase services.

Qualifications

• Bachelor’s degree in a field related to developmental disabilities is required.
• Master’s degree preferred.
• 3 years’ experience working with individuals with developmental disabilities.
• Bilingual (Spanish to English) preferred.

Responsibilities

• Use person-centered planning to assist clients in accessing public and private resources before using regional center funds.
• Monitor vendor services and ensure services align with authorizations.
• Evaluate effectiveness of Individual Program Plans and coordinate community resources.
• Conduct annual reassessments with the interdisciplinary team; perform client assessments as needed.
